Definition

A biomarker is something measurable — a blood level, a scan — used as a stand-in for an outcome that is harder or slower to measure.

A biomarker is something measurable — a blood level, scan, physical sign, or defined characteristic — used to show what is happening in the body. Researchers often choose one because the outcome they care about takes longer or is harder to observe. The catch: moving the marker does not automatically improve that outcome.

What is a biomarker?

A biomarker is an objective measurement that indicates a biological process, disease process, or response to treatment. Blood glucose, blood pressure, hormone concentrations, and tumor size on a scan can all qualify. That is the practical biomarker meaning: a finding that gives researchers information without asking whether someone feels or functions better.

Biomarkers can diagnose disease, help select trial participants, track safety, or show that a compound reached its target. The FDA definition draws a firm boundary: a biomarker is not itself a measure of how a person feels, functions, or survives.

Is a biomarker the same as a surrogate marker?

A surrogate marker is a biomarker used in place of a direct clinical outcome, but not every biomarker earns that job. Hemoglobin A1c (HbA1c), for example, is a validated surrogate endpoint for microvascular complications in diabetes in a defined setting. Validation requires evidence that the stand-in predicts clinical benefit.

That context matters. FDA warns that even a validated surrogate can miss benefits or harms outside the pathway it measures. A lower laboratory value is a result; fewer complications is a different result. The endpoint glossary explains how trials choose between them.

Is IGF-1 a biomarker of peptide results?

IGF-1 is a biomarker of growth-hormone-axis activity, not automatic proof of muscle gain, recovery, or longer life. Sermorelin can raise growth hormone and IGF-1 in people, so a higher IGF-1 biomarker shows that the hormonal pathway responded. The outcome claim still needs its own measurements: strength, body composition, daily function, or another result people care about.

The same caution applies to GDF-11, where blood-test interpretation is part of the scientific dispute. Ask whether a test measured the intended molecule cleanly and whether that measurement predicts the claimed benefit.

How should you read a biomarker result?

Read a biomarker result as evidence for exactly what was measured, then stop before adding an unmeasured benefit. Ask whether the marker was merely associated with the outcome, whether it is a validated surrogate for this specific use, and whether the study also measured how people felt or functioned.
