What does systemic mean for a drug?

An oral tablet can be systemic after absorption from the gut. Intravenous administration puts a compound directly into circulation. A subcutaneous injection starts in the fatty layer under the skin, then may be absorbed into the blood. Different entrances, same basic destination: exposure beyond one small treatment site.
Systemic describes exposure, not proof of benefit. Detecting a compound in blood shows that the compound got there; it does not prove a meaningful health outcome or safety with repeated use. Pharmacokinetics—what the body does to a drug—is not the same as clinical effectiveness.
What is the difference between systemic vs local treatment?
Systemic vs local describes where a treatment is intended to act and how widely the body may be exposed. A local treatment aims at one area: a cream on a patch of skin, an injection into a joint, or drops in one eye. A systemic treatment is meant to circulate or produce effects away from the administration site. The axis is about reach, not whether the treatment is “strong” or “weak.”

A joint injection may leak some drug into circulation; a skin product may cross the skin barrier. “Mostly local” is often more accurate than “never systemic.” That detail is where useful pharmacology begins and tidy marketing language starts to wobble.
Is systemic vs topical the same distinction?
Systemic vs topical is related, but topical describes where a product is applied, not an absolute promise that none reaches the blood. FDA defines topical administration as application to a particular spot on the body’s outer surface. FDA separately defines transdermal administration as passage through the skin into systemic circulation. A patch can therefore sit on the skin while deliberately delivering a whole-body effect.

Broken or diseased skin may behave differently from intact skin. Covering a product can also change absorption.
That nuance prevents a common mistake: “topical” does not automatically mean zero systemic exposure, while “systemic” does not automatically mean dangerous. The honest question is how much reaches circulation under the actual conditions of use, followed by what that exposure does.
Why does GHK-Cu make the difference concrete?
GHK-Cu shows why the same molecule in a serum and a syringe should not be treated as the same evidence package. The GHK-Cu profile separates topical cosmetic use from injected research use. Its topical evidence concerns skin products and local skin outcomes. Those findings do not establish the benefits, dose, or long-term safety of systemic injection.
The route changes more than convenience. A serum places GHK-Cu on the skin barrier, where penetration is a live research question. The distinction also protects the evidence tier. A topical human study remains evidence about that topical product, formulation, population, and outcome.
