Titration

Definition

Titration means starting at a low dose and raising it in small steps, giving the body time to adjust and revealing side effects early.

Titration is the practice of ramping a dose up gradually instead of jumping to the target amount on day one. You start low, hold there for a stretch, then step up, hold again, and keep climbing until you reach the intended dose — or the point where the benefit and the side effects balance out.

Think of it like easing into cold water rather than diving in. The slow approach gives your body time to acclimate, and it surfaces problems while the dose is still small and easy to back off from.

The most familiar example right now is GLP-1 drugs. Semaglutide and tirzepatide are famous for nausea when started too high, so their schedules deliberately begin at a low dose and increase every few weeks. That ramp is titration, and it’s the main reason people tolerate these drugs at all.

For a concrete example, a semaglutide schedule may pause at its current step when nausea becomes hard to manage, then resume only after symptoms settle; moving ahead automatically would miss the point of titration. Tirzepatide uses the same logic: each planned increase is a checkpoint, not a race to the highest listed dose. In both cases, the response at the current dose, especially side effects and tolerability, determines whether the next step is to hold, rise, or move back down. The calendar provides the framework, but the body’s response supplies the signal in real time that makes titration useful rather than mechanical.

Titration cuts both ways: you can titrate up to find an effective dose, or titrate down to come off something gently. The underlying logic is the same — change the dose in small steps, give the body time between steps, and let how you actually respond guide the next move rather than rushing to a number on a chart.
