Intramuscular injection

Definition

An intramuscular injection goes deep into a muscle, where rich blood flow absorbs it faster than a shot under the skin.

Intramuscular — usually shortened to “IM” — means into a muscle. This injection uses a longer needle to reach past the skin and fat and deposit the liquid into muscle tissue, in a large muscle like the shoulder (deltoid), thigh, or glute.

The reason to go deeper is speed. Muscle is packed with blood vessels, so a compound placed there is picked up and carried into the bloodstream faster than one left in the shallow fat layer of a subcutaneous shot. It’s the difference between dropping a dye into a fast-moving stream versus a still pond.

That faster uptake is why many vaccines and some hormone preparations go IM. It can also handle a slightly larger volume than a subcutaneous injection, and it suits compounds formulated as oily depots that release over time.

For peptides specifically, subcutaneous is the more common route — but a handful are given IM, and knowing the difference matters. IM uses a longer needle, a 90-degree angle, and careful site selection to avoid nerves and blood vessels, so it’s a bit more technique-dependent than a subcutaneous shot. The trade-off is simple: IM is faster in, subcutaneous is slower and gentler.

Cerebrolysin is a concrete example: its manufacturer permits the prepared solution to be given as an undiluted IM injection in small volumes. In that use, the needle carries the solution through the skin and fat, then releases it in muscle instead of the fatty layer used for a subcutaneous injection. The word intramuscular tells you where the injection lands; it does not describe the mixture’s mechanism, purpose, or effect after it enters circulation. It also does not make IM the default for every peptide: the right route depends on the specific formulation and its instructions, which matter more than a general rule about peptides.
