Steady state

Definition

Steady state is the dosing plateau where the amount of drug entering the body matches the amount cleared, so average blood levels stop rising.

Most drugs with predictable, first-order clearance take about four to five half-lives to reach steady state. The NIH pharmacology reference puts levels at roughly 94%–97% of the plateau by then. A one-week half-life points to four or five weeks.

The steady state half-life rule assumes consistent dosing and clearance. Missed doses, dose changes, or altered kidney or liver function can move the curve; each dose change starts a climb toward a new plateau.

What does steady-state concentration actually look like?

Steady-state concentration is a repeating range, not a ruler-flat line. Levels still rise after a dose and fall before the next; the peaks and troughs repeat instead of ratcheting higher. The steady state meaning in drug research is “stable average exposure,” not “one concentration every minute.”

This belongs to pharmacokinetics, the study of how a compound enters and leaves the body. Dose and clearance determine plateau height; half-life mainly determines the climb time.

Why does semaglutide take weeks to level out?

Semaglutide shows the long-half-life problem in real life: its elimination half-life is about one week, and the Ozempic label says steady-state exposure arrives after four to five weeks of weekly dosing. The current Wegovy label escalates injectable doses every four weeks to reduce gastrointestinal reactions.

Semaglutide can work before that plateau. The point is narrower: early weeks do not show the full exposure of a fixed dose, and an increase resets the clock. Does steady state mean a drug is working?

Steady state proves only that input and clearance have balanced under that dosing pattern—not benefit, safety, or the right dose. A measured concentration is pharmacokinetic evidence; whether symptoms, weight, blood sugar, or another outcome improves needs human clinical evidence. That distinction is the useful answer to what is steady state in pharmacology—and the part most definitions leave out.
