Lactoferrin is an approximately 80 kDa iron-binding glycoprotein found in milk and human secretions. Human trials give the anemia case real weight, though with important limits; the broad “immune boost” pitch is much less settled. In the United States, bovine lactoferrin is sold over the counter as a dietary supplement, not an FDA-approved drug.

What is lactoferrin?

Lactoferrin is a protein your body already makes, not a small research peptide in a vial. Human lactoferrin appears in breast milk, tears, saliva, airway fluid, and immune-cell granules; most capsules contain bovine lactoferrin purified from cow’s milk. UniProt records the human precursor as a 710-amino-acid protein under P02788. Its sugar groups help push the finished glycoprotein to roughly 80 kDa.

That size matters. A lactoferrin supplement is digested like a food protein, although the intact protein and smaller fragments may remain biologically active in the gut. PubChem searches return named lactoferrin fragments, not a clean record for the whole glycoprotein, so this profile does not borrow a fragment’s CID and pretend it identifies the full protein.

How does lactoferrin work?

Lactoferrin works first as an iron manager: each half of the protein can hold ferric iron tightly. Picture a two-handed clamp taking loose iron off the table. That can deny microbes a nutrient they need, while lactoferrin-related signaling may also affect hepcidin, the hormone that controls whether iron stays stored or enters circulation.

Lactoferrin also binds some microbial surfaces and can be cut during digestion into smaller antimicrobial peptides. That biology makes the immune peptide category a sensible home, but a plausible defense mechanism is not proof that swallowing a capsule prevents infections. LL-37 offers a useful contrast: both belong to innate defense, yet their structures, evidence, and supplement markets are very different.

What lactoferrin benefits have human evidence?

Lactoferrin benefits are best supported for iron-deficiency anemia, not for a vague upgrade to the whole immune system. A 2022 systematic review and meta-analysis included 11 lactoferrin-versus-ferrous-sulfate intervention studies: nine randomized and two nonrandomized, with 1,262 participants. Lactoferrin favored hemoglobin, serum iron, and ferritin despite lower measured fractional iron absorption.

The catch is not small print. Most participants were pregnant women, several studies lacked blinding, and heterogeneity reached 96% for hemoglobin and 98–99% for iron and ferritin. The signal is decent, but “better than ferrous sulfate” is not yet a universal conclusion for men, nonpregnant women, children, or every cause of anemia. Lactoferrin iron effects may partly come from lowering inflammation and hepcidin rather than simply delivering more iron.

Does lactoferrin boost the immune system?

Lactoferrin has real immune biology, but general lactoferrin immune claims run ahead of adult clinical outcomes. A human systematic review found that lactoferrin did not reduce respiratory-infection incidence in adults: the pooled odds ratio was 1.00. A child subgroup showed fewer infections, but study heterogeneity was high, so age and formulation may matter.

The much-discussed 2025 randomized trial assigned 66 healthy adults to 0.34 g/day or 3.4 g/day recombinant human lactoferrin, or 3.4 g/day bovine lactoferrin, for 28 days. Its primary question was whether participants developed anti-lactoferrin antibodies; it found no alloimmunization signal or excess adverse events. Useful safety evidence, yes. Proof of fewer colds or stronger immunity, no.

What should you know about a lactoferrin supplement?

A lactoferrin supplement usually contains bovine lactoferrin, while recombinant human lactoferrin is a distinct ingredient and should not be treated as interchangeable. Iron saturation also matters: apo-lactoferrin carries little iron, while holo-lactoferrin carries more. Labels rarely explain the difference well, which makes comparisons between products and trials messier than the front of the bottle suggests.

The 2025 safety trial studied far larger amounts than many retail capsules, but that does not convert 3.4 g/day into a recommended dose. An NIH supplement-label record, for example, lists a 250 mg bovine product; a retail serving and a trial intervention answer different questions. Is lactoferrin safe? What are the risks?

Lactoferrin appears reasonably well tolerated in short oral trials, but long-term safety is not mapped with drug-level precision. The 66-adult trial found no excess adverse events through 28 days, and the iron studies generally describe better gastrointestinal tolerance than ferrous sulfate. Those findings do not settle years of use, pregnancy outside the studied settings, or every recombinant and bovine formulation.

Milk allergy is the clearest practical risk. In 2025, the FDA posted a recall of 300 mg apolactoferrin capsules because milk was not declared; a severe milk-protein allergy can be life-threatening. Iron disorders also deserve medical context because lactoferrin changes iron handling rather than acting like an ordinary multivitamin.

Lactoferrin is legal to sell in qualifying U.S. dietary supplements and foods, but no lactoferrin supplement is FDA-approved to treat anemia, infection, or immune deficiency. FDA does not preapprove supplements for safety and effectiveness the way it approves drugs; the NIH regulatory summary explains that split plainly. “Sold over the counter” and “approved medicine” are not synonyms.

Bovine lactoferrin also appears in FDA food-ingredient records, including infant-formula uses, while recombinant human products have their own product-specific history. As of July 2026, lactoferrin is not named on WADA’s current prohibited list, though tested athletes still face the ordinary contamination risk attached to supplements. The dated regulatory reference is the place to re-check if either rule changes.
