A third kind, theta-defensins, is functional in some monkeys but not in people, where the gene is switched off. Because “defensins” is a whole family rather than one molecule, there is no single formula or sequence that stands for all of them — each member has its own. How do defensins work?
Defensins work mostly by physically breaking microbes open. That electrostatic attraction is why defensins hit bacteria, fungi and some enveloped viruses but largely spare your own cells, whose outer membranes are wired differently. What does the research show?
Defensins are one of the most-studied peptide families in all of biology — a PubMed search returns more than ten thousand papers, thousands of them in humans — but the honest headline is that almost all of that work is observational and mechanistic, not “we gave people defensins and measured a result.” The evidence splits cleanly into three buckets, and keeping them apart is the whole point:
- As built-in defense (human-observational): defensins are a documented part of human innate immunity, and disturbed defensin activity travels with real diseases — for example, reduced Paneth-cell alpha-defensins are repeatedly reported in ileal Crohn’s disease (PubMed). These are associations in people, not proof that adding defensins fixes anything.
- As a diagnostic signal (human-observational): measuring alpha-defensin in joint fluid has been studied and marketed to help detect prosthetic joint infection, with diagnostic-accuracy studies in patients (PubMed; ClinicalTrials.gov NCT03714165). This is the clearest human-tested use — but it is defensin as a readout, not as a therapy.
- As an administered therapy (mechanistic-hypothesis, none-in-humans): the idea of dosing defensins or synthetic mimics to fight infection is biologically reasonable and active in preclinical work, but the one registered human trial of a defensin-mimetic (ORTD-1 for COVID-19 pneumonia, NCT04708236) was withdrawn before enrolling. There is no human efficacy or safety result to report.
So defensins are simultaneously extremely well-studied and unproven as a thing you would take — a distinction the internet tends to blur.
Is it safe? Side effects
For defensins the safety answer depends entirely on which question you are asking. Made by your own body in the right place at the right time, defensins are not just safe but essential. Taking defensins — or buying something sold as them — is a different matter, and there is essentially no human safety data for it.
