Somatropin is recombinant human growth hormone: a prescription biologic with strong human evidence for diagnosed growth hormone deficiency and several pediatric growth disorders. It is not an approved anti-aging drug, is prohibited in sport, and U.S. law creates an unusually serious criminal penalty for knowingly distributing it outside authorized medical use.
What is somatropin?
Somatropin is the drug name for recombinant human growth hormone, often shortened to rhGH or HGH. The protein has 191 amino acids, weighs about 22,124 daltons, and has the same amino-acid sequence as the main growth hormone made by the pituitary gland. A somatropin injection therefore supplies the hormone itself; it does not ask the pituitary to release more.
That distinction separates somatropin HGH from the compounds grouped in the growth-hormone hub. Products such as Norditropin are ready-to-use pens, while Genotropin includes powder-and-diluent cartridges. Both are regulated prescription medicines, not generic “peptide vials” with borrowed branding.
How does somatropin work?
Somatropin binds the growth hormone receptor and starts signals that change growth and metabolism, both directly and through insulin-like growth factor 1 (IGF-1). Think of growth hormone as the instruction and IGF-1 as one of the work orders sent downstream. In children with open growth plates, that pathway increases bone growth; in adults it affects protein synthesis, fat use, fluid balance and glucose handling.
The mechanism explains both benefit and risk. The same signaling that can restore growth or lean mass in deficiency can also retain fluid and reduce insulin sensitivity when exposure is too high.
What does the human evidence show?
Somatropin has human randomized-trial evidence for replacement in true growth hormone deficiency, plus FDA-reviewed evidence for several pediatric growth disorders. The current Norditropin label covers pediatric GHD, Turner syndrome, Noonan syndrome, small for gestational age (SGA), idiopathic short stature and Prader-Willi syndrome, as well as adult GHD. Labels differ by brand, so approval is product-and-indication specific.
In adults with diagnosed GHD, randomized trials show less fat mass and more lean mass, but those findings do not automatically transfer to healthy lifters. A 166-person placebo-controlled trial found body-composition changes without significant improvements in strength, endurance or quality of life (Hoffman et al., 2004). Hormone replacement corrected a deficiency; that is a narrower claim than “HGH builds performance.”
Does HGH work for anti-aging?
HGH for anti-aging has human trial data, but the result is not rejuvenation. A systematic review of 18 controlled study populations in healthy older adults found about 2.1 kg less fat and 2.1 kg more lean mass on average, with no significant weight change and more edema, joint pain, carpal tunnel syndrome and glucose problems (Liu et al., 2007).
Somatropin is not FDA-approved to reverse normal aging. Lean mass also includes water, so a scan moving upward is not the same as stronger muscle, better function or longer life. This is a clean example of a measurable biological effect outrunning the sales pitch.
What are the main somatropin side effects?
Somatropin side effects most often reflect fluid retention and metabolic spillover: swelling, joint or muscle pain, tingling, headache and impaired glucose tolerance. The Genotropin label also warns about intracranial hypertension, pancreatitis, worsening diabetic retinopathy, hypothyroidism and tumor progression or recurrence in susceptible patients.
Somatropin is contraindicated during acute critical illness after certain surgery, major trauma or respiratory failure; with active malignancy; and with active proliferative or severe non-proliferative diabetic retinopathy. Pediatric cautions include closed growth plates and severe obesity or respiratory impairment in Prader-Willi syndrome.
Is HGH legal in the United States?
Somatropin is legal in the United States as an FDA-approved product prescribed for authorized medical use, but the answer to “is HGH legal?” changes outside that lane. Somatropin is not scheduled as a controlled substance. Yet 21 U.S.C. §333(e) makes knowing distribution, or possession with intent to distribute, for human use outside the statute’s authorized disease-or-recognized-condition pathway a federal felony offense punishable by up to five years in prison and fines. If the offense involves someone under 18, the maximum is ten years.
The statute expressly defines human growth hormone to include somatropin, somatrem and their analogues. A conviction is treated as a felony Controlled Substances Act violation for forfeiture, even though HGH itself is not scheduled. FDA also says biologics are not eligible for 503A/503B compounding exemptions, so “compounded HGH” is not a workaround. This legal gap helps explain the market for sermorelin, ipamorelin and MK-677: they stimulate GH signaling rather than distributing HGH itself. Their approval and evidence problems do not disappear; the statute is simply written around human growth hormone.
How are somatropin and long-acting HGH used?
Daily somatropin and weekly long-acting forms are subcutaneous prescription injections, with dose chosen by diagnosis, age, body weight, response and IGF-1. As one label example, Norditropin reports 0.17–0.24 mg/kg/week for pediatric GHD and an adult non-weight-based starting range of 0.15–0.3 mg/day. Those are cited label doses, not a personal protocol.
Three long-acting relatives reduce injection frequency. Sogroya contains somapacitan, Ngenla contains somatrogon, and Skytrofa contains lonapegsomatropin, a prodrug that releases somatropin. All are once-weekly U.S. products, but their approved ages and conditions differ. “Long-acting HGH” is a family description, not permission to swap pens or doses.
Is somatropin banned in sport?
Somatropin is prohibited at all times under class S2 of the 2026 WADA Prohibited List, in and out of competition. The ban also reaches GH analogues, fragments and releasing factors, including the three long-acting forms above and the secretagogue side of the market. A legitimate diagnosis may support a therapeutic use exemption; a prescription alone does not erase anti-doping rules.
