Follistatin is a natural protein that neutralizes myostatin, the body’s built-in brake on muscle growth. Animals engineered to make more follistatin grow visibly bigger muscles. In people, follistatin is not FDA-approved: the only human data come from small gene-therapy trials, and the injectable peptide sold online has no efficacy trial behind it.

What is follistatin?

Follistatin is a protein your own body makes — a single-chain glycoprotein first isolated from ovarian follicular fluid, where it was found to suppress the hormone FSH (that job is where the name comes from). Its more famous role showed up later: follistatin binds and switches off myostatin, the signal muscles use to stop themselves growing too large. The research products people buy under names like FS-344, FST-344, FST-315 and FST-288 are attempts to bottle that muscle-growth angle. Follistatin is not a medicine you can be prescribed for muscle building and not a legal supplement — in the United States the peptide version exists only as a research chemical, while the gene-therapy version exists only inside clinical trials.

How does follistatin work?

Follistatin works by taking a brake off muscle growth. Muscle tissue makes myostatin (also called GDF-8), a TGF-β-family signal whose whole job is to tell muscle “that’s enough, stop growing.” Think of myostatin as a governor on a car’s engine — a limiter that caps the top speed on purpose. Follistatin is a clamp that pins that limiter down: it physically binds myostatin (and the related signal activin) so they can’t dock with their receptors, and with the brake disabled, muscle is free to grow past its usual ceiling (follistatin research index, PubMed). The honest catch for anyone buying a vial: neutralizing myostatin only builds muscle if enough follistatin actually reaches the muscle and stays there — and that is exactly the part injecting a short-lived protein struggles with.

What does the research show?

Follistatin’s evidence splits cleanly into “strong in animals” and “barely tested in the way people actually use it.” In animals, raising follistatin builds muscle — but the eye-catching results come from gene therapy and genetic overexpression, where the follistatin gene is switched on inside the muscle so the tissue makes the protein continuously, not from a subcutaneous shot of the peptide. In humans, the only interventional data come from small early-phase gene-therapy trials: a follistatin gene delivered directly into the muscles of patients with Duchenne muscular dystrophy (NCT02354781), and with Becker muscular dystrophy and inclusion-body myositis (NCT01519349). Those trials were small and largely uncontrolled, and their main achievement was showing the localized gene approach was tolerated — not proving a dependable strength gain. A separate Phase 1 tested an injectable follistatin plasmid gene therapy (NCT06411366). Notice what is missing from that list: a human trial of the plain follistatin peptide for muscle growth in healthy people. There isn’t one. So for the reason most people search this compound, the grade is honestly animal-only, no human study — and the reason that matters is timing: the human work here is running through gene therapy first, so the peptide’s real test is still ahead of it, not behind it.

What the community reports

Outside the lab, follistatin has a following among bodybuilders and myostatin- inhibitor enthusiasts chasing the “myostatin knockout” look they have seen in photos of hyper-muscular animals. Reported use centers on subcutaneous injections of a reconstituted follistatin peptide over a few weeks. These accounts are anecdotal — not controlled data, heavily filtered by survivorship bias (people who see nothing tend to stop posting), and impossible to separate from the training and other compounds usually running alongside. They also can’t tell you whether the vial contained active, correctly folded follistatin at all, which for a large protein sold on the research-chemical market is a real unknown. Useful for understanding the interest; not useful as proof it works.

Is follistatin safe? Side effects

Follistatin’s honest safety answer is “we don’t really know in humans.” There is almost no human safety data on the injectable follistatin peptide; the human safety information that exists comes from small gene-therapy trials, where the localized approach was generally tolerated over the study window — reassuring for that specific delivery, but not a clean bill of health for a peptide you inject yourself. The theoretical concerns follow from the biology: follistatin also binds activin and touches pathways involved in reproduction, inflammation and cell growth, so broadly suppressing them is not obviously free of consequences, and no long-term human data exist to check. As with any research-market vial, the real-world risk skews toward the source — purity, correct protein folding, dose accuracy and sterility are only as good as the seller behind a given batch, and for a large protein those are harder to get right than for a short peptide.

Follistatin is not FDA-approved, and it is not a legal dietary supplement. In the United States, follistatin gene therapy is investigational — you can only receive it inside a registered clinical trial — while the follistatin peptide sold online is an unapproved research chemical, marketed “for research use only.” No approved follistatin drug product exists in the FDA’s database (FDA drug information). How is follistatin used, and can it be mixed?

Follistatin peptide is sold as a lyophilized (freeze-dried) powder that has to be reconstituted with bacteriostatic water before use, like most research peptides. Hype vs reality

Follistatin’s biggest myth is that injecting the peptide reproduces the “double-muscled” animals from the famous photos. It almost certainly doesn’t — and this is the single most important honest point on the page. Those animals were genetically engineered to overproduce follistatin (or knock out myostatin) from the inside, continuously, for life. A subcutaneous shot of follistatin protein is a completely different situation: the protein is large, it’s cleared quickly, and getting it to sit on myostatin inside muscle tissue is exactly the delivery problem gene therapy exists to solve. So the fair read is: the biology of myostatin inhibition is real and promising, the animal-genetic results are dramatic, and the injectable-peptide version is the least-supported link in the chain. Believe the mechanism; be skeptical of the vial.

Does follistatin work to build muscle in humans?

Follistatin has not been shown to build muscle in healthy humans. How do people take follistatin?

In the research and community setting, follistatin peptide is reconstituted with bacteriostatic water and injected subcutaneously. That’s the reported pattern, not a recommendation — and it comes with the honest caveat that a large protein like follistatin may not reach muscle in useful amounts when injected this way, which is a big part of why its human muscle-building record is empty.

Is follistatin the same as a myostatin inhibitor?

Follistatin is one kind of myostatin inhibitor — it works by physically binding and neutralizing myostatin (and activin) so they can’t signal. Other approaches block the myostatin receptor or use antibodies instead. Who is follistatin for — and who should be cautious?

Follistatin draws in people chasing maximum muscle — the myostatin-inhibitor crowd who saw the hyper-muscular animal photos and want the same switch flipped in their own body. The honest framing: the mechanism is real and one of the more exciting stories in muscle biology, and human work is underway through gene therapy. But the injectable-peptide version that’s actually for sale is the weakest-supported form of it — no human efficacy trial, thin safety data, a real delivery problem, and an unregulated product where even protein quality is a question mark. Anyone drawn in by the animal photos should keep the strongest results (genetic overexpression) mentally separate from the product they’d buy (a peptide shot), and anyone pregnant, managing a hormone-sensitive condition, or unwilling to accept genuine unknowns has every reason to wait for the human data to arrive.
