Dulaglutide (brand name Trulicity) is an FDA-approved, once-weekly injectable GLP-1 receptor agonist for type 2 diabetes. Unlike most of the research peptides on this site, dulaglutide has been tested in large human randomized trials: it lowers blood sugar, trims a few kilograms of body weight, and in one major trial cut cardiovascular events. Dulaglutide is a prescription drug, not a research chemical.

What is dulaglutide?

Dulaglutide is a lab-made version of GLP-1, a hormone your gut releases after you eat, engineered to last far longer than the natural version. GLP-1 on its own breaks down in minutes; dulaglutide fuses two GLP-1-like chains onto a fragment of a human antibody (an IgG4 Fc region), which makes the whole molecule big — roughly 60 kilodaltons, huge for a peptide — and slow to clear. That size is the trick: dulaglutide sticks around for about five days, so one injection covers a whole week. It is sold under the brand name Trulicity by Eli Lilly and comes as a ready-to-use single-dose pen — no vial, no mixing, no reconstitution.

How does dulaglutide work?

Dulaglutide works by switching on the GLP-1 receptor, the same receptor your own gut hormone uses to tell the body “food has arrived.” Think of GLP-1 as the body’s after-dinner signal. When dulaglutide activates that receptor it does four useful things at once: it nudges the pancreas to release insulin, but only when blood sugar is high; it dials down glucagon, the hormone that pushes sugar up; it slows how fast the stomach empties, so glucose enters the blood more gently; and it quiets appetite in the brain, so you feel full sooner. The insulin-only-when-needed part is why dulaglutide rarely causes low blood sugar by itself — it isn’t forcing insulin out around the clock, it’s amplifying the body’s own mealtime response.

What does the research show?

Dulaglutide sits in the rare corner of this site where the evidence is settled, not emerging: it has been through many human randomized controlled trials, the gold standard. For blood sugar, the AWARD program of trials showed dulaglutide lowered HbA1c — a three-month blood-sugar average — by roughly 1 to 1.5 percentage points, holding its own against or beating comparator diabetes drugs (PubMed record set). The headline result, though, is cardiovascular. In REWIND, a randomized, placebo-controlled trial of about 9,900 adults with type 2 diabetes, dulaglutide cut the risk of major cardiovascular events — heart attack, stroke, or cardiovascular death — by about 12% over a median of 5.4 years (Gerstein et al., Lancet 2019). Weight loss is real but modest: a few kilograms, and a bit more at the higher 4.5 mg dose — helpful, but dulaglutide is a weaker weight tool than semaglutide or tirzepatide, and it is not FDA-approved for obesity. Is dulaglutide safe? Side effects

Dulaglutide’s safety answer is unusually well-grounded: it has years of trial and real-world data behind it, and for most people the side effects are annoying rather than dangerous. The common ones are gastrointestinal — nausea, diarrhea, vomiting, abdominal pain and reduced appetite — usually worst when you start or step up the dose, then easing over weeks. The label also carries a boxed warning about thyroid C-cell tumors: in rats and mice, dulaglutide and other GLP-1 drugs caused these tumors, and whether that happens in humans is unknown, so dulaglutide is contraindicated in anyone with a personal or family history of medullary thyroid cancer or MEN 2. Rarer but serious risks flagged on the label include pancreatitis and gallbladder disease. Dulaglutide seldom causes low blood sugar on its own, but the risk climbs when it is combined with insulin or a sulfonylurea, and those doses often need lowering. Dulaglutide is a fully FDA-approved prescription drug — one of the few compounds on this site with no regulatory gray area at all. The FDA first approved dulaglutide (as Trulicity, Eli Lilly, BLA 125469) for type 2 diabetes in 2014; in 2020 it added the cardiovascular risk-reduction indication and the higher 3.0 mg and 4.5 mg doses. Dulaglutide is legal to prescribe and dispense through a pharmacy with a prescription, and it is approved for children aged 10 and up with type 2 diabetes as well as adults. One honest boundary: dulaglutide is not approved for weight loss on its own the way semaglutide’s Wegovy is — using it purely for weight management is off-label. How is dulaglutide dosed and taken?

Dulaglutide is taken as a single subcutaneous (under-the-skin) injection once a week, on the same day each week, with or without food. Because it ships as a prefilled, ready-to-use pen, there is nothing to reconstitute or measure — which is part of why it became popular. The FDA-labeled doses start at 0.75 mg once weekly and can step up to 1.5 mg, then 3.0 mg, then 4.5 mg once weekly, with the higher doses added to squeeze out a bit more blood-sugar and weight benefit. These figures are the doses reported in the trials and on the label, not a personal protocol — dulaglutide is a prescription drug, and the right dose is a decision for you and a clinician, not a number to copy off a webpage. Dulaglutide vs semaglutide and tirzepatide

Dulaglutide, semaglutide and tirzepatide are all once-weekly injectables in the same family, and they are the three most-compared names in the GLP-1 world. In short: dulaglutide is a pure GLP-1 agonist with a strong cardiovascular record and gentler weight loss; semaglutide is a more potent GLP-1 agonist that drives larger weight loss (and, as Wegovy, is approved for obesity); tirzepatide is a dual GIP/GLP-1 agonist that produces the biggest weight loss of the three. Does dulaglutide actually work?

Yes, for what it’s approved to do. But dulaglutide is a prescription medicine for a reason — the boxed thyroid warning, the pancreatitis and gallbladder risks, and the GI side effects are real. Anyone with a personal or family history of medullary thyroid cancer or MEN 2 should not take it, and anyone chasing weight loss alone has better-studied options in the same family.
